Forms 10 and 12 are issued by Companies House and must be completed and returned with the Memorandum and Articles of Association and a cheque for £20.
The purpose of form 10 is to enable you to appoint your first director and secretary of the company, as well as the address of the registered office.
Form 12 is a Statutory Declaration that must be completed by the director or secretary of the company and signed in the presence of a Solicitor or a Commissioner of Oaths (This should cost you no more than £10).
